MyPatientScheduler - Online Appointment Scheduling

If you can't remember your MyPatientScheduler username, the system can look it up and email it to you.

Follow these steps to recover a forgotten username:

1. Navigate to the main login page.
2. Click the I forgot my Username link.
3. Enter your email address and tab to the next field. The system will look up your secret question and prompt you for it.
4. Enter the security code, your secret question, and press Submit. The system will email your Username to the email address registered with your account.
